Mouth: slightly weak with maximum effort; IV (moderately severe dysfunction), gross: obvious weakness and/or disfiguring asymmetry. At rest: normal symmetry and tone. Motion forehead: none. Eye: incomplete closure. Mouth: asymmetric with maximum effort; VI (total paralysis), no movement.
